I have a really good template I was given by an executive recruitment consultant many years ago.
It works really good in paper form as it's 4 A4 pages that you copy out on one A3 sheet and fold in half. Hardly ever failed to get me through the door.
Anyone wants to PM me they can have it, I've just updated mine funnily enough.
Another thing is think about making small changes for each individual application - sometimes pays off.
